Heavy Vehicle Drivers

Since 2006, a heavy vehicle driver's traffic violations and accidents are registered in his or her driving record. The file gives an overall portrait of a driver's conduct on the road.

To receive a copy of a driving record, drivers must follow the procedure outlined under the heading "Driving Record Request".

Any other person who wants to obtain a copy of a driving record must complete the form Authorization for the disclosure of a driving record by the Société de l'assurance automobile du Québec (PDF: 162 Kb - 1 page; dynamic form). Enclose it with the request.
