Information the SAAQ Requires

General Information

• First and last name at birth
• Date of birth
• Current and previous address
• Home telephone number
• Telephone number where you can be reached during the day or where a message can be left for you
• Date your new address takes effect

Depending on your situation:

• If you hold a driver's licence
  • driver's licence number (file number)
  • reference number on your driver's licence
• If you do not hold a driver's licence
  • licence plate number of your vehicle
  • registration certificate number of your vehicle
• If you have a claim file with the SAAQ
  • claim number
•  If your vehicle is jointly owned
  • by mail or to an SAAQ service outlet
    • first and last names at birth of joint owners. At a service outlet, only one of the joint owners needs to come with the relevant information (no power of attorney is necessary)
    • joint ownership file number
    • vehicle registration certificate number 
    • vehicle licence plate number
    • current and previous address
    • telephone number where one of the joint owners can be reached during the day or where a message can be left
    • date the change of address takes effect
    • signature of one of the joint owners
• If your vehicle is registered under the name of a business
  • by mail or at an SAAQ service outlet
    • Form: Avis relatif au changement d'adresse de l'entreprise
      (PDF: 99 Kb - 1 page; French only)
      OR
    • letter on the business’s letterhead or bearing the official seal
      • SAAQ file number or subdivision number or Québec Enterprise Number (NEQ)
      • previous address
      • complete new address
      • date the change of address takes effect
      • first and last names of the employee authorized by the business
      • function of the employee authorized by the business
      • date of the request
      • signature of the employee authorized by the business
